Cottonwoods(CTW) Long-Term Care Facility is home to 221 LTC residents; the majority of whom are in multi-resident rooms, grouped into units of 32-60 occupants. CTW is facing critical challenges including inadequate physical space and related issues that affect resident and client comfort, visitor access, staff safety, and infection prevention and control (IPAC) measures. Specifically, the 1975 building has a deteriorating sanitary system that cannot be repaired without major impact to current residents and operations. The Project will include a “household” model that reimagines three core elements of a traditional nursing home: the physical environment, philosophy of care, and workforce model. The goal of a household is to create real community within a space that residents recognize as home. Residents will be able to directly access the kitchen, outdoors, well-protected private spaces, and comfortably shared spaces. Residents will live on their own terms, according to their rhythms and daily choices. Staff are empowered to support and maintain the philosophy of person-centered care, including upholding residents’ choices and needs for care.
